From connecting people to connecting their care.
Ursamin helps clinics track and manage their patients’ changing health needs —so care teams can respond earlier and help patients get the care they need.
The next step matters
For our founder, Shannon Aylesworth, this mission is personal. During her mother’s care, she saw how missed communication across providers could leave a family trying to piece together what should happen next.
After losing her mother as a result of disconnected treatment plans, Shannon founded Ursamin to help clinics follow through on the care patients need.
Infrastructure experience applied to care
Shannon brings more than 25 years of telecommunications experience helping complex systems work at scale. Ursamin applies that experience to care coordination—using technology to make patient information usable and care easier to manage.
The growth in patients living with multiple chronic conditions has changed what outpatient care requires: more regular engagement, more coordination across providers, and more follow-through. We believe technology can help clinics meet these demands without relying on more staff for every new need—moving from trying to keep up to delivering the care patients need today.
